Q:  How do I know if my plan covers massage therapy?   A:  If you are uncertain about insurance coverage for massage therapy, please contact your insurance company.  Prior to your appointment, these forms can be mailed to you along with a detailed Insurance Verification Form upon your request.  Otherwise, here are some basic questions to help you find answers about your coverage: Does your coverage include massage therapy treatments? Are there any limitations (i.e. monetary or number of visits annually)? Do you need prescription or referral from your Doctor or HMO to receive massage treatment? Are you required to make a co-payment or will there be a coinsurance amount that is your responsibility?  If so, what is the amount or percentage?

Q:  What if my insurance is not on your credential list? A:   A reimbursement receipt for payment at the time of service can be provided for those patients out of network. Q:  How do I pay for my first visit? A:  If you don’t know the type of coverage concerning your policy, Co-pay or co-insurance can be determined after billing for first dates of service.
